Help needed with Thank you Card!

Hello. Sorry if this is posted in the wrong area but I am making a thank you card for my coach and have no idea what to write. I am moving in the summer to a new town and am therefore saying thank you and goodbye in a sense. She has been my coach for the last 10 years...

Here is what I was thinking:

Thank you for changing my life. You are an inspiration on and off the ice. Thanks for 10 wonderful years together (insert coach name here) love (insert my name here).

It cannot be that long as the card is kind of small.. Any thoughts comments appreciated thanks.

Moving away doesn't mean you have to lost complete touch with your coach. I've moved away from one coach and had another coach move away and we keep in touch via email. They kinda like to hear that there's still progress being made, especially if they've been working with you for 10 years. It's nice to hear the fruition of all the hard work you've done together.
